This claim form is for parents/carers/young people that have been authorised by the Local Authority to receive a Personal Travel Budget (PTB). A personal travel budget is a contribution towards the cost of travel to support families to make their own arrangements for a child or young person’s journey between home and school. Please Note: Claim forms must be submitted by the last day of each month and will be processed within two weeks. If you do not receive your payment within two weeks, please email TransportRequests@stoke.gov.uk
