About this form

You can use this form to tell us about problems with roadworks carried out by a third party organisation (known as a statuatory undertaker).

In the first instance we ask you contact the organisation that has completed the works, as they may be able to quickly resolve the issue themselves. If you haven't had a satisfactory response from them, complete this form and we will investigate.

If the issue has not been caused by roadworks, please use this form to report a highways problem instead
