Use this form to:
- Apply for a free concessionary bus pass
- Renew an existing concessionary bus pass
- Request a replacement concessionary bus pass (a £8 fee will be taken at the end of the form)
- Request a companion to be added to your existing concessionary bus pass (a £15 fee will be taken at the end of this form) If the correct document is not attached the application will be rejected.
Any residents who do not have internet access can use public computers in libraries, where library staff are able to assist in the application process. Residents who wish to go to a Customer Service Centre can make a face-to-face appointment by calling 01782 234234 or book an appointment online.
