The Quality Assurance Form is managed by a central team for adult social cares services within Stoke-on-Trent.
This form can be used by all, whether you use a care service yourself, are the loved one of someone who uses a care service, a resident or a healthcare professional.
Not only has this form been created to include everyone, it will also help our Quality Assurance Team to be able to work closely with private, public and voluntary sectors.
Please note: This feedback form should be used to advise the Commissioning and Quality Assurance Team of any quality issues about the services being delivered.
Under no circumstances is this form to be used to refer adult safeguarding issues, these should always be referred through the normal process at 0800 561 0015.
