We are a data controller for the personal information we hold about you in relation to HealthWorks Stoke-on-Trent.
The data will be used to:
- contact you with information about the programme
- invite you to upcoming events and training opportunities
- monitor, evaluate and audit the impact of workplace health and wellbeing interventions at your organisation
- monitor, evaluate and audit the impact of HealthWorks Stoke-on-Trent on health and wellbeing outcomes for workers in Stoke-on-Trent
- assess whether your organisation is eligible for accreditation and to award successful organisations accordingly
We are able to do this because we already have your consent to do it. You have provided this consent:
- by completing the application to join the HealthWorks Stoke-on-Trent network
- by requesting to be part of the HealthWorks Stoke-on-Trent network and providing your contact details
- by requesting to be kept informed of news and information by HealthWorks Stoke-on-Trent
Data protection law provides you with certain rights, but not all of these rights will be available to you in all situations. Where we are under a legal duty to use data for a particular purpose you will not have the right to prevent it being used in that way.
- you can ask to see the information we hold about you
