When you speak to an administrator at the Living Well Hub Service (delivered by Stoke-on-Trent City Council) we will collect the following information about you:
- name
- gender
- address
- age/date of birth
- telephone number
- email address (if you have one)
- what service you are interested in (help to quit smoking, smokefree homes, healthy eating, physical activity, weight management or drugs and alcohol)
We only keep this data for the time specified in our retention schedule or as allowed by law. We use this data to:
- help you to decide which service you can access
- forward to the provider which delivers the service you have expressed an interest in
- monitor the number and types of contacts made to the Living Well Hub
We will only forward your details to the relevant service provider when you have given consent for us to do this.
When leaving your contact details on the Living Well Hub voicemail (0800 085 0928) or via the Stoke-on-Trent City Council website we will use this information to make contact with you.When we make contact with you we will collect the same data as described above.
We only keep this data for the time specified in our retention schedule or as required by law. We use this data to:
- help you decide what services are available for you to access
- forward to the provider which delivers the Service you have expressed an interest in
- monitor the number and types of contacts made to the Living Well Hub
We will only forward your details to the relevant service provider when you have given consent for us to do this.
If we cannot get in touch with you or you don’t respond to our follow up we will only keep your data for the time specified in our retention schedule or as required by law.We will use this data to monitor the number and type of contacts made to the Living Well Hub.
When you are referred to the Living Well Hub Service (delivered by Stoke-on-Trent City Council) by a health professional or front line member of staff the following information will be collected:
- name
- gender
- address
- age/date of birth
- telephone number
- email address
- what service you are interested in (help to quit smoking, smokefree homes, healthy eating, physical activity , weight management or drugs and alcohol)
- if you are pregnant: the name of your midwife or the midwife making the referral and your carbon monoxide reading (if this has been taken during a midwife appointment).
- if you have additional requirements for example language, hard of hearing, disability
- that you have consented to the referral (note if you have not consented to the referral your referral will not be processed) and you have been informed the reason for the referral and how your data will be used.
We only keep this data for the time specified in our retention schedule or as allowed by law.
We use this data to either- contact you to discuss the services available or
- forward to the provider which delivers the service you said you were interested in. The provider we pass your details onto depends on the service you have chosen and where you live/work/are registered with a GP.
We also use your data to monitor the number and type of contacts made to the Living Well Hub.

If you would like to withdraw consent please notify the referrer or the Living Well Hub on 0800 085 0928 who will safely dispose of the information.
You should be aware that Stoke-on-Trent City Council has a duty to protect public funds.
We may therefore use the information we have for the prevention and detection of fraud.
We may share this information with other bodies for these purposes.
We may also share this information with other parts of the council or other relevant organisations for purposes which may include enforcement.
